Type 9 jumping out of gear

Hi guys, got a bit of a problem with a Type 9 box.

Occasionally jumps out of 3rd and 4th when under heavy load. Not good!!

It is a Quaife straight cut box.

I have stripped it down but cannot find anything wrong!

Any suggestions guys??

mine (T9, 4x4 box fitted to std case abd shortened input shaft) was jumping out or 3rd.
Took it to BGH, diagnosed as "dogs" (I think that is what he called them)needed grinding, cost £5 a gear or something. On top of std rebuild costs...
They have sort of coffin shaped cutouts that hold the gear, but wear a bit allowing it to jump out.

Sorry for vagueness! It is why I paid them to do the job.
