Yes, you do need to fill in and send V627/1 (build up report)...... You will need a receipt for the chassis if you didn't build it yourself......when my car was inspected (before sva) and given the chassis number, it was this form that the inspector used to check my engine number etc......

Well that's how it was in 2004!
