MK Pedal Box

Hello, could someone tell me what comes with the MK £58 pedal box. Three pedals or two? Is it floor mounted?
Dont suppose anyone has a pic of it?

I think it's 3 floor mounted, welded to chassis, at lest that's how the MK indy's ones are..
