I've used body filler paste(suppose you could also use JB Weld or similar) on a couple of gear-knobs.
Wrap the gear-lever in cling-film making sure it comes quite a long way below the thread.
Whack some mixed filler into the knob, trying to make sure it goes in all the way, then place it over the gear-lever, lining it up how you want it.
Simples....
HTH.

8mm nylon bar and drill it...

Heat gear lever with small torch. Smear hot melt Glue on Lever, fit knob over the top and position. Leave to cool.

I did this with a Pool 8 ball, it never moved in 6 years.
Also easy to remove by re-heating the lever again.

Do NOT put HMG in the hole in the ball / knob, it cools too quickly when it hits the cold lever.
This works well and is easy to remove.

Thanks for the replies to date,

but all I'm after is the plastic insert, as I still want to be able to remove it quickly if needed.
