Question15: Axle weights (Got car weighed today)
Actual results = Front 300kgs Rear 290kgs.. What should I put on form? Was thinking 400kgs front 400 kgs rear?

If you made the car entirely yourself, then
Q2: Whatever you want
Q3: Whatever you want
If you've made a kit (MK or whatever) then you may have to put that name down.

Might be worth clearing it with the local DVLA man. As mine was built from scratch, I called it a Lathyrus 7 - don't ask why - it's a longish story!.

Q15: 400kgs sounds OK - you'll get a chance to correct it at the test if you're too far wrong.
