V55/4 or V55/5?

Which form do I need to fill in for the DVLA? V55/4 or V55/5? The car uses donor parts from 2 different vehicles and is scratch built rather than from a kit. Oh, and when can I download them from?

You can also collect it from the DVLA office if there is one near you, it's one of those "self carbon" paper sets which is why you cannot download it - BTW - get two so that when you mess up the first - you have a spare
