SVA Form

Hi,

Can anyone tell me do i only have to fill in upto Question 17?

I've got the stupid paperwork head on!

Any other things i should be sending with the form apart from the £150 cheque

Yes the questions after that dont apply dont forget to sign and date form.Nothing else to go with form just the fee.You normally will be sent another short questionare on receipt of sva form just fill in (did you build the car your self/for your self etc) and send that back.
