vin number

trying to get a vin number/inspection organized,got a form through (inf148) asking me to send my license/passport/birth cert and utility bill to the ministry of lost post.
the rest of the form dosnt look like the one i really need,anyone else had this/what form you use?

Chassis/VIN

I seem to remember sending off a few bit and pieces to the VRO, mainly to confirm who i was and where i live.

Sent it with prepaid/stamped A5 envelope , was all returned with a chassis number slip a few (maybe nearly 10 days) later.

Not sure about driving licence but i did send pics of build and complete car plus reciept and for big parts and utillity bill (council tax)

My ID was checked at the time of inspection, supplied my VIN.
