Threads, washers and reaming uprights

Cortina uprights.
My upper bj does not protrude enough to provide visible threads so I have some questions:
Has anyone tried grinding out the tapered hole using valve paste and a tool made from an old transit bj?
Do you have to have a washer on the upper bj?
Is this a common problem or am I wierd (a search reveals lots of issues with maxi lower bjs but the upper ones don't seem to be mentioned) ?
For suspension and shock bolts do you use a spring washer or a flat washer?

I have cortina uprights, and it's a perfect fit with the Transit TRE's. Conus width 16mm if I remember correctly.

I use flat washers and Nyloc nuts on the suspension, so no need for spring washers.

Probably the ball joint at fault, some are a bit short . ISTR someone wrecked an upright trying to adapt it to fit the ball joint. Better to get another joint.
