Hmmmm..
not usually known for heads going.

No.3 piston and bore is the usual.

Or the main bearings due to oil over heating or starvation.

as a rule these engines are reliable,
they dont like miss-use and lack of servicing.

Any particular version/year or does that apply to all subaru flat 4 engines ever made?

Our local engineering firm had a head off one to repair, cracked between valve seats and the bloke wanted it welding.
