Is toe in/out achieved bij screwing the extensions in or out of the track rods or by screwing the track rods in or out of the steering rack?
It depends on the steering arrangement but speaking for the escort rack, the track rods are fixed to the rack - they can rotate and swivel - so to bring the font of the wheels in nearer t han the rear (toe-in) you screw in the track rods into the track od ends -both are RH thread. (same for sierra I believe)

When you're doing the initial rough setup then you screw the ball joints back & forth until you're reasonably close. Then you tighten
everything up.
After that, you rotate the steering arm to get the setting exactly right. Once it's correct, make sure that the ball joint is in the centre of
its travel, and tighten the lock nut. You have to watch the rubber boot when you're rotating the steering arm, as sometimes it tries to curl
up.

Conventional method is to screw/unscrew the track rod ends to adjust.
If you have extensions on the rack I'd leave them as is so as not to weaken that connection unduly.
