The Sayano-Shushenskaya dam accident

here

someone says:

Looks like they accidentally closed the outlet doors on no. 2 turbine rather suddenly when it was running at full power. You cannot stop such a huge volume of water so suddenly and the massive increase in pressure blew the turbine apart and launched it skywards. The escaping water tore away the roof and flooded the hall, when it shorted out all the transformers. All of the other turbines that were operating suddenly had no electrical load on them, and accelerated to destruction. the internal explosions of the windings etc caused them to to sieze in their housings, and the very sudden stop caused them to twist their housings out of the floor .

A properly designed control system wouldn't let them make dangerous control decisions (at least without levels and levels of override required).

Very bad

Surprising as before the accident that place looked in very good condition indeed and not the usual run down accidents waiting to happen as we tend to expect to see over there. Pity someone seems to have made a mistake hopefully it will be repaired and they’ll learn from their mistakes. It will be interesting to hear what went wrong.
