Hi,does the engine have to come out to do the sky cam

Yep
You talking about a crossflow I take it.

engine out turned remove rockers, remove push rods
turn engine upside down remove dizzy remove sump and front cover plate.
disconect timing chain cam shaft should then come out.
you may have to change the followers check with sky 1st.

AND oil pump off (when distributor out/off) as that's driven by the cam shaft too.

Also when getting cam followers (and Camshaft) make sure they are the RIGHT diameter as they are different early (thinner) and late (thicker). I know because I found out the hard way after I thought SHELL TUNE in Northampton had supplied me the right ones for the engine and I didn't check. I asked for cam and followers for a late engine and they gave me ones for the early engine. It went bang 800 miles later.

If you ARE talking about a crossflow engine - it's about as complicated as Meccano, so quite easy to work on as long as you keep your brain in gear...
