Long shot but any one know what this means

At the risk of teaching you to suck eggs, and with the caveat that this is knowledge from Asterisk not 3CX....

The first thing an IP phone will do is REGISTER with the PBX which tells it where it can be found on the network (ie it's IP address), what capabilities it has, etc. The PBX will return information like if there are any voicemails waiting so the phone can put it's message waiting indicator on.

I'm assuming that the message is an authentication failure from one of your phones trying to do that which I guess means either you've got a phone on the network that isn't configured on 3CX or someone has 'fat fingered' a username or password somewhere along the lines of setting that phone up.

That what I thought but the phones (softphones) are Autoprovisioned.....
