I haven't read this book but I'm familiar with David Vizard's work know it to be generally excellent.

About the port size, the first question any tuner has to ask is 'what purpose will the engine be put?' i.e. race, road, mostly race and some road etc etc. If a fair proportion of road work is envisaged then wide airways will be a problem due to the low gas speeds at lower rpm which lead to poor cylinder filling and poor swirl which means low torque production.
