Rear shock come apart after rebuild!!!

After getting my Westfield's rear shocks refurbed a couple of months ago I took my car for its first 'shakedown' today only to by confronted by a rattle when going over bumps. On retuning to my drive a removing the wheels I discovered the entire unit has com apart and is considerably damaged! What do you think the cause was and how I should approach the people who rebuilt them?

Looks like the cap has unscrewed itself? Although the cap threads seem to have no peak, so has it been torn out?

Shocks too short maybe? If they've been rebuilt maybe the thread was damaged during rebuild and couldn't with stand rebound on the road

The shock absorber is nearly always what limits suspension travel so cannot really be too short.

Damage or incorrect assembly when overhauled would seem probable if this was the first use after overhaul. I'd contact the company concerned, I'm sure they'll agree to help.
