What engine & starter type.

If it is an xflo with an inertia 2 bolt starter then it could be caused by the sandwich plate between engine and gear box being omitted when the engine and gearbox were mated up.

A poorly fitted starter on my 2lt zetec has chewed a secton of teeth out of the ring gear.
May be worth taking the starter off and looking at the ring gear
