Yes download it and fill it in. It's not as bad as it looks. This is how we filled in for our MK Indyblade:

Page 1 Q1 to Q8 is straightforward.
Page 2 Q9 = A
Page 3 Q10 leave blank
Page 4 Q11 to Q15 Straighforward, Q16 leave blank, Q17 Straightforward, Q18 and Q19 leave blank
Page 5 Q20 leave blank
Page 6 leave blank
Page 7 sign and date

Pages 8 to 12 are just notes.

where do these forms come from, your local DVLA office ? also can you tell engine's age from donor's vin plate ?

VOSA and DVLA are different, you get the forms from VOSA - see their web page (above).

No car VIN isn't enough, because the engine could have been changed. You need the engine number, often the manufacturer (is yours a Ford?) will be able to send you a letter confirming the date of engine manufacture for an admin fee.
